RTHM
The RTHM Intelligence platform (for record analysis) is available to individuals aged 16 and older. However, clinical services, including prescriptions and enrollment in the RTHM Clinic, are strictly limited to adults aged 18 and older.
You must be physically present in a state where RTHM is licensed at the time of your clinical service or when submitting medication requests. For RTHM Clinic membership, we also require that you be a permanent resident of a licensed state due to the nature of ongoing treatment.
RTHM can help patients with medication access who are located in the following states: California, Colorado, Florida, Georgia, Illinois, Maryland, Massachusetts, New York, North Carolina, Ohio, Oregon, Texas, Virginia, Washington. More states to come.

Yes. As long as you're physically located within one of our active states, you are eligible for treatment, labs and consultations.
RTHM can only offer you consultative services and prescription refills in a state you're currently located in.
RTHM is accessible through the main RTHM web application, which is optimized for both desktop and mobile devices. You can access all features through your web browser on any device.
Signing up and interacting with the RTHM intelligence platform is free. You can access the world's most knowledgeable AI for complex chronic conditions at no cost. When you decide to level up your care by ordering a medication or lab via RTHM or joining the RTHM Clinic, you will be prompted to pay for the services.
At this time, RTHM does not accept Medicare or Medicaid.
RTHM Platform
RTHM does not currently support the direct parsing of raw genomic data files (such as .txt or .csv files). To include genetic information in your analysis, please upload interpreted PDF reports from your testing company or raw data files for the AI to reference specifically in the 'Genetics' tab.
RTHM currently supports direct integration with Garmin and Oura devices. You can connect these by navigating to the 'Wearables' section of your dashboard. Integration for Apple Watch and other devices is currently on our development roadmap.
To ensure medical accuracy and security, these identity fields are locked once populated via the identity verification process (Stripe) or retrieved from your official medical records. To update these, you must contact RTHM support for manual verification.
Suspected diagnoses are conditions flagged by RTHM's analysis of your questionnaires or records as potential matches. These are listed for your review; you can manually confirm them if they are established diagnoses or dismiss them if they are inaccurate.
RTHM's intelligence solution is particularly focused on infection-associated chronic conditions like Long COVID, ME/CFS, MCAS, and POTS and can provide useful insights for related complex chronic illnesses.
RTHM's intelligence platform is specifically built for patients with infection-associated chronic conditions. You can still use RTHM's intelligence platform with a few limitations. The first one is that we cannot pull your medical records from the Health Information Exchanges (HIEs) outside of the US so you will need to upload your records manually. The platform is HIPAA compliant at this time - not GDPR, etc. Additionally, we do not currently offer any care options outside the U.S. (e.g. treatments, lab orders, and the RTHM Clinic).
RTHM's new intelligence system is open to everyone—no clinic subscription required!
Our system is not set up to sufficiently process genetic data yet, but it is a feature we are working on. If any genetic data is uploaded, the system may not parse or interpret those results accurately or correctly, so view the interpretation with caution. If you do upload genetic data, please let us know how the system did with it - it will help us as we develop this feature.
Currently, the automated medical record connection performs a one-time retrieval of your historical data. It does not automatically refresh in the background. To incorporate new results, you must either disconnect and reconnect your portal or manually upload the new lab reports as PDFs to the 'My Documents' section.
To delete your account, log in and navigate to the 'Security' tab within your Profile settings. Scroll to the bottom and select 'Delete Account.' This will remove your profile and personal data from our systems, though some clinical assessment records must be retained for legal compliance.
RTHM's automated retrieval system typically pulls electronic health records from the past 5 years. If you have older records that are relevant, please upload them manually as PDFs in the 'My Documents' section.
Your health data is stored securely and used to provide personalized insights and suggestions. We do not sell your data to third parties. Your data is only used to power the features of RTHM and improve the service. You can review our full privacy policy for more details.
Your health information is only accessible to you and RTHM's intelligence system. You can choose to share your health plans or specific insights and suggestions with your care team through the Care Team Sharing tools. RTHM staff might have limited access for tech support and customer service purposes but all access is logged and monitored. Additionally, you have the ability to delete your data anytime in your profile page.
If you choose to electronically pull your health records through the health information exchanges (HIEs), we are required to provide information back to the HIEs in return so your current and future clinicians can access it. The information we share back to the HIEs includes prescriptions and visit notes related to medications and services you order through RTHM as well as medically qualified questionnaires (except those for psychiatric conditions) that you answer. If you choose to connect your records via the health information exchanges (HIE), we are required to provide some data back. We do so by sharing questionnaire information that is not about mental health conditions with the exchanges and where your other care providers can see them.
Yes, going into your RTHM profile and selecting "Delete Account" will remove all personally identifiable data from the platform. See the consent for more detail about how we use your de-identified data.

Yes. To manage processing costs, RTHM has daily usage limits for the AI chat. If you reach this limit, your access will reset at midnight UTC. You can optimize usage by starting new chat sessions for different topics rather than continuing one long thread.
Your chat history is automatically saved. You can view previous conversations by clicking the history/menu icon in the top corner of the AI chat interface, allowing you to resume past sessions or retrieve previous summaries.
You can manually upload documents at any time in the 'My Documents' section. Once uploaded, you can visit the 'My Labs' or 'Encounter Notes' section to manually edit the date, provider name, or report title to ensure your medical history timeline is accurate.
RTHM uses a passwordless login system for enhanced security. Enter your registered email address on the sign-in page to receive a one-time 6-digit verification code in your inbox each time you log in.
You can connect your medical records through the Medical Records Integration feature. This process involves authorizing RTHM to access your records through health information exchanges. Step-by-step instructions are provided in the application.
No, you don't need to complete all the questionnaires. You can choose which questionnaires are most relevant to your health concerns. However, completing more questionnaires provides a more comprehensive assessment and can lead to more accurate insights and suggestions.
A Health Roadmap constitutes an analysis of your health care and generation of potential root causes and treatment/lab suggestions. Most patients complete their Health Roadmap protocol in under 10 minutes. But that depends on how much health data each person has.

If you notice a transcription error or if the AI misinterprets a lab value, please report it to support or mention the discrepancy in the AI chat. Our team will review the original documents and correct the data in your chart to ensure the analysis remains accurate.
Individual PDF files or images should be under 100MB. If you encounter an 'upload failure' with a large file, try splitting the document into smaller increments (e.g., 50 pages at a time) and uploading them separately.
Check your spam folder first. Some privacy or forwarding services (like DuckDuckGo Email Protection) may block our links. We recommend adding 'no-reply@rthm.com' to your safe senders list or using a standard email provider to ensure delivery.
Questionnaire results are based on standardized scoring systems. If you feel the results don't accurately reflect your experience, you can retake the questionnaire or discuss the results with your healthcare provider. Remember that questionnaires are screening tools, not diagnostic instruments.

RTHM Medications
RTHM Direct offers LDN in standard capsule strengths of 1.5mg, 3.0mg, and 4.5mg. If you require highly customized titration (such as ultra-low doses starting at 0.1mg) or specific compounded formulations, you will need to work with a provider through the full-service RTHM Clinic.
RTHM typically requests that compounding pharmacies use dye-free veggie capsules with minimal fillers, such as methylcellulose. These formulations are also sugar-free. If you have specific allergy concerns, you can verify the exact inactive ingredients with the pharmacy before your order is processed.
Currently, RTHM Direct only prescribes Metformin for the treatment of acute COVID-19 (prophylaxis or immediate post-exposure). It is not offered as a treatment for established Long COVID symptoms through the Direct service, though it may be managed within the RTHM Clinic.
You can book a medication appointment with your provider from your dashboard.
If you are a patient using RTHM's direct prescribing or lab ordering services, you can reach out to your provider through our contact form. Your message will be routed to your provider who can answer your question through encrypted email communication.
Unfortunately, we don't allow changing the information in the intake form for regulatory reasons. If you believe you were unfairly disqualified from getting a treatment, please contact our customer support through your dashboard.
Generally speaking, yes. Generic medications have the same active drug as the name brand, but may have some different inactive ingredients. Generic drugs are usually far less expensive than brand name. If you know that you tend to tolerate brand name drugs better than generics, prefer the brand name, or are already stable on the brand name and wish to stay on it -- please write that in the free text notes before you submit your prescription request.
RTHM currently offers the following medications: LDN, Metformin, Ivabradine, Oxaloacetate, Ketotifen, Pyridostigmine, Propranolol and Midodrine. More medications to come soon, including Cromolyn and Guanfacine.

Yes. While RTHM has preferred mail-order partners like Infuserve America for compounded needs, you can request that your prescription be sent to local pharmacies or services like Amazon Pharmacy or Mark Cuban Cost Plus Drugs during your assessment.
If an item is on backorder, RTHM will notify you of the estimated restock date. If an item is permanently out of stock with no restock date, RTHM will cancel the order and issue a full refund to your original payment method.
RTHM Direct does not use automatic renewals or subscriptions. If you wish to stop a medication or skip a refill, simply do not submit a renewal assessment. No charges will be processed unless you manually initiate a request.
RTHM relies on Stripe for our payment processing needs. This external service provider is recognized for achieving the industry's highest security standards. Stripe adheres to top-tier practices for the storage, decryption, and transfer of card details. Rest assured, all personal data is safeguarded with the utmost security measures.
RTHM uses external vendors to negotiate and provide discount cards that can be used at the pharmacy of your choice. Oftentimes, using the discount card provided, patients can obtain an even lower price than they would if they used their insurance.
Yes. On your patient dashboard you will be able to see the status of your assessment and whether the prescription has been sent to your pharmacy. Additionally, you will be receiving an email whenever the clinician has reviewed your health assessment and sent a prescription to your pharmacy.

Most medication flows will provide patients with 90 days worth of supply, although some of them will require the patient to check in with updated symptoms, vitals, side effects before refills are sent.

Providers review every request for safety. A prescription may be denied if your reported vital signs (like heart rate or blood pressure) fall outside of safe ranges, if there is a potential drug interaction with your current medications, or if the treatment requires more intensive in-person monitoring than our remote service provides.
To ensure your safety, RTHM generally recommends starting only one new medication at a time. We typically require a 30-day waiting period between starting new treatments to allow you to accurately monitor your body's response and any side effects for each specific drug.
For medications requiring titration, we often issue an initial 30-day 'up-taper' supply. After three weeks, you complete a progress questionnaire, and a second prescription is then issued for the remaining 60 days of the maintenance cycle at no additional assessment cost.

RTHM's medications service does not require a direct consultation with a clinician at this time. A clinician will review the information you have provided in your assessment and decide whether a medication is appropriate for you. If you would like to consult with the clinician, you do have the option to book a medication consultation anytime you get an approved medication from RTHM.
Please note, we may add new medications to RTHM's direct medication service in the future that require a telehealth consultation--this will be clearly indicated on the medication page and in the medication flow.

No, RTHM does not require a referral. That said, our services are in no way a replacement for your primary care provider, in fact, we request that you notify your primary care of any new treatments you start from RTHM so they can stay in the loop on your treatment.
Once you submit your assessment for a specific medication, a clinician will review your information and assess whether the medication is appropriate for you. If the clinician deems this medication appropriate, they will send the prescription to your pharmacy of choice for regular medications, or to our partner mail order compounding pharmacy for compounded medications.

No. Your intake form responses will give the clinician the information they need to prescribe a treatment for you. If our providers need additional information, we will reach out to you.
Identity verification must be completed 'live' via smartphone. If the system rejects your ID, ensure you are in a well-lit area and using a physical card. If you are physically unable to complete it or have a non-binary gender marker that causes a block, contact support for manual verification assistance.

Yes. A clinician safety assessment is required every 90 days for ongoing prescriptions to monitor vitals and check for new drug interactions. This assessment carries a fee (typically $49–$59) that is separate from the cost of the medication at the pharmacy.
No. RTHM Medications is a cash-pay service and does not accept insurance directly for assessments or compounded medications. For standard medications, you can use the RTHM discount card (CareCard) at retail pharmacies to lower your out-of-pocket costs.
You can submit a claim to your insurance provider, but we can't guarantee they'll reimburse you.
RTHM's direct medication service is designed as cash pay and provides a discounted cash pay coupon code for you to use at your pharmacy for non-compounded medications. While some patients do find that their insurance ends up covering the medication at the pharmacy, please do not expect that to happen. We do not work directly with or guarantee coverage of medications by your insurance.
We also do not complete prior authorizations for medications prescribed through this lower touch prescribing service. Prior authorization requirements are decided by your health insurance provider. You can check with your pharmacist or health insurance provider to find out if your prescribed medication is covered by your insurance.

Your payment method will be charged only if/ when one of our clinicians has reviewed and approved your prescription request. If your request is not approved, you won't be charged. A hold for the amount of the charge is put on your method of payment to reduce the risk of a payment being declined - that hold is removed if your prescription request is not approved.

RTHM accepts all major credit and debit cards (Visa, MasterCard, American Express, Discover, etc.) via Stripe.

RTHM's safety assessments range from $49 to $59 and are only charged if the clinician approves your prescription request, which covers the prescription and support for 90 days.

Yes, we accept patients who developed Long COVID symptoms after vaccination as well as after infection! We are interested in the mechanisms behind "vaccine long haul" and how they are similar and different from "infection-induced long haul." At RTHM, we feel it's our duty to help the people who got the vaccine to help protect themselves and those around them and then suffered ill effects.
No, RTHM does not replace your primary care physician. You are required to have a primary care physician while you work with RTHM.

All appointments with your RTHM Clinic team are virtual/telehealth visits. You may need to go to a nearby lab to have blood (and possibly urine or saliva) tests done, go to an imaging center, or visit certain specialists as recommended by your RTHM provider. Your RTHM Clinic team will work with you to locate a specialist, imaging center, lab draw location, or arrange for a mobile phlebotomist to come draw your labs at your home for a fee.
You can, but it is A LOT of blood to have drawn at one time, so we generally order it in segments and have patients get a smaller volume of blood drawn every 1-2 weeks, especially if you have post exertional malaise (PEM or PESE), tend to get lightheaded easily, or know that you tend to become more symptomatic after blood draws.
If you are anemic, iron deficient, or get very ill with blood draws, please let your RTHM provider know at your first visit. There are some lab companies that can run more labs on less tubes of blood, but they do not accept insurance. Using such a lab may be a better option for you.

You will have access to our patient portal, your health plan summary from each provider visit, your health history summary, health metrics, vital signs, lab and imaging results. Research data used for diagnostics development is not available for download until we have published it. You will have the ability to download your clinical data and/or share it with your providers outside of RTHM (we encourage this!).
We cannot prescribe any controlled substances or any medication that requires an in-person exam prior to prescribing. You will need to have any controlled substances prescribed by one of your local medical providers.

While RTHM is cash-pay, the specialized lab testing we order is fulfilled by third-party facilities. PPO plans are much more likely to cover these out-of-network labs. Without a PPO, these diagnostic costs can exceed $10,000, which may hinder your clinical progress.
No, not at this time. We are also unable to accept patients with Kaiser Permanente, VA or TriCare, Medicare, or Medicaid (MediCal in CA) health plans. These plans either do not legally allow patients to pay cash for parts of their care or do not generally cover the labs, imaging, procedures, referrals or treatments prescribed off label by clinicians not in their networks.

Your Flexible Spending Account (FSA) or Health Spending Account (HSA) can be used to pay for many of the costs associated with your care through RTHM, including the RTHM Clinic membership costs. Please see the details of your health plan's HSA or FSA for more information.
Yes! RTHM can provide you with superbills for your visits if you wish to try to get some of the costs reimbursed on your own through your out-of-network insurance benefits.
This answer depends on your individual insurance plan, as each has different laboratory testing coverage policies & rates. Our providers order labs through Quest Diagnostics because Quest is contracted with most major PPO insurance plans. If Quest is a contracted lab for your insurance plan, most of the labs ordered by your RTHM provider through Quest should be covered, but please check your individual insurance plan (since Quest is the one that bills your insurance for the labs, not RTHM). Your deductible and co-pays may still apply to lab costs.
